How to refer

Fax the referral to (818) 806-6282 or call our office at (818) 745-1157, Monday to Friday, 10:00 AM to 4:00 PM. You can also email office@ha-homecare.com to arrange a referral, but please send patient records by fax, not by email.

What to include

• Patient name, date of birth, address and phone, and a family contact
• Insurance: Medicare, Medicare Advantage, Medi-Cal or commercial, with member ID
• The home health order, with the disciplines needed (skilled nursing, PT, OT, speech) and the reason for care
• Recent history and physical or hospital discharge summary, and the current medication list
• The face-to-face encounter note, or the date it took place

Missing something? Send what you have. Our office will contact you for the rest and confirm the patient's insurance and any authorization before the first visit.

What happens next

A clinician visits the patient at home, completes the assessment and sends you the plan of care to review and sign. You receive updates when the patient's condition changes, and our team coordinates with you throughout the episode of care.

Patients we care for

Post-surgical and post-hospital patients, wound care, IV therapy, medication management and teaching, chronic disease management (including heart failure, diabetes and COPD), and physical, occupational and speech therapy. Our staff speaks English, Spanish, Armenian, Russian, Farsi and Tagalog.
