Registered Nurse (RN), Home Health

HA Home Care is seeking a skilled and compassionate Registered Nurse to provide in-home patient care and support. You'll perform Start of Care and Recertification visits, develop and update individualized care plans, deliver skilled nursing interventions, and coordinate care with physicians and the interdisciplinary team.

Key Responsibilities

• Complete Start of Care, Recertification, and Discharge visits in compliance with Medicare Conditions of Participation
• Perform comprehensive assessments and develop individualized Plans of Care
• Provide wound care, medication management, IV therapy, and disease-management support
• Communicate with physicians regarding changes in condition and implement orders promptly
• Document all visits and assessments accurately and on time
• Serve as Case Manager for your assigned caseload, coordinating with PT/OT/ST/MSW/HHA
• Supervise Home Health Aides every 14 days
• Participate in on-call rotation as assigned

Requirements

• Active California RN license
• Graduate of an accredited nursing program
• Current CPR certification
• Valid CA driver's license and reliable, insured transportation
• Minimum 1 year of home health, public health, or acute care experience preferred
