Licensed Vocational Nurse (LVN)

HA Home Care is looking for a compassionate Licensed Vocational Nurse to provide direct patient care under the supervision of a Registered Nurse. You'll follow individualized plans of care, deliver treatments, and work closely with our interdisciplinary team to help patients recover safely and comfortably at home.

Key Responsibilities

• Provide direct patient care as defined under California's Nurse Practice Act
• Implement nursing care following comprehensive assessments and the plan of care
• Provide accurate, timely documentation of all patient services
• Educate patients and families on diagnosis, self-care, and the plan of care
• Coordinate with the interdisciplinary team on patient needs
• Support pain and symptom management, monitoring patient response to treatment

Requirements

• Graduate of an accredited practical/vocational nursing program
• Current, active LVN license in California
• Minimum 1 year of nursing experience; home health or medical/surgical experience preferred
• Valid driver's license with insured, reliable transportation
• Strong verbal, written, and organizational skills
