Intake Coordinator / Office Manager

North Hollywood, CA · Full-Time · On-Site · Mon–Fri, 10:00 AM–4:00 PM · $23–$26/hour

HA Home Care is looking for an experienced Intake Coordinator / Office Manager to lead our front-end referral and intake process while keeping day-to-day office operations running smoothly. This role is central to how quickly and accurately new patients get the care they need.

Key Responsibilities

  • Receive, screen, and process new referrals from hospitals, physicians, SNFs, and families
  • Perform real-time insurance verification, prior authorizations, and reauthorizations
  • Track physician orders, plan-of-care approvals, and recertification signatures through completion
  • Assign new patients to case managers and coordinate timely Start of Care
  • Maintain accurate, same-day documentation in Data Soft Logic (DSL)
  • Build and maintain strong relationships with referral sources
  • Oversee daily office operations: scheduling, phones, supplies, and vendor coordination

Requirements

  • Minimum 3–4 years of home health intake or coordination experience, with hands-on EMR experience. At least 2 years of Data Soft Logic (DSL) experience strongly preferred; candidates with experience on similar systems (Homecare Homebase, WellSky/Kinnser, Axxess, etc.) are encouraged to apply.
  • Experience with insurance verification, authorizations, and referral management
  • High school diploma required; Bachelor's degree preferred
  • Proficiency with Microsoft Office, Teams, and Adobe Acrobat
  • Excellent organization, multitasking, and communication skills; multilingual a plus
